Filling the Gap
When children become adults, they are often expected to navigate through life with little or no support. This is especially difficult for many adults living with Autism Spectrum Disorder (ASD). The lack of autism-friendly care available leaves many struggling to connect in their communities.
To help fill this gap, we have created a much-needed new service for adults with ASD in partnership with the Glenrose Rehabilitation Hospital, the Sinneave Family Foundation and other agencies in the community.
TRANSITIONS provides autism-friendly support, health care and mental health services for adults with ASD.
This hub of support includes:
• access to autism-aware family doctors.
• support services from a caring team.
• connections to other services within the community.
• access to our in-house psychiatrist who has expertise in ASD.
What You Can Expect
We’ll talk with you to determine what your needs are and then schedule a meeting.
You'll meet with your mental health coordinator and your new family doctor. You are welcome to bring a family member or support person with you.
If needed, we can refer you to our PCN psychiatrist. We can also connect you to community resources and help complete applications for funding and services.

You may be transitioning from your pediatrician or family physician, need a diagnosis, or you’re just trying to find an autism-friendly physician. Whatever the reason, we can help.
You do not need a doctor’s referral to be part of TRANSITIONS.
