Edmonton Oliver Primary Care Network's Pharmacist Led Hospital Discharge Program was the first program of its kind when it was introduced in 2009. Our pharmacist works with a family physician and other providers to reduce the risk of adverse events for patients who are discharged from a hospital. Read about our results and lessons learned.
